He messed up by leaving Khan on Ceti Alpha V, he messed up by ignoring Saavik citing regulations to raise shields, he was embarrassed to appear fallible by putting on glasses to read, he admitted to cheating on the Kobayashi Maru test, and only Spock's sacrifice saved them all from destruction.
